Call to confirm the Patient. Advise Patient they will need social security number, valid FL driver's license, valid phone number, and access to an email account. They also need to bring a copy of a current utility bill or proof of residence in the event validation is required by FL Site.
Check in Patient as normal, but collect payment based on the fee schedule below.
Print Consent Form for Patient to fill out. The form will follow the patient into the exam room.
Bring Patient to the Exam Room and Follow Normal MA Process with exception to HEDIS.
Fill out New Application with Patient. Requires Dr. Carolyn Maldonado's Login. Click Here to Link to Site.
Wait for Email to arrive to Patient.

Complete Registration with Patient. An Email will arrive to patient with login credentials. This step will require patient to have credit card for FL State registration payment.

Within a few minutes Patient will receive confirmation of their Medical Marijuana Card Status. This will include a digital copy of their card. The Email will arrive with (No Subject) as the subject line as of the time of writing. 8/22/22
Let the Doctor know that the application is complete so the office visit can continue, and Rx can be provided. Depending on Approval by FL State.
At check-out book patient for renewal of license appointment 7 months in the future.
